Every practice in Shropshire now has a Community Care Coordinator. At Cleobury Mortimer Medical Centre this is Katja Jones. Her role here is to help patients needing support or advice by signposting them to appropriate services in the NHS, Council or voluntary sector.

Katja's working hours at the practice are Tuesday and Thursday 10am - 4.30pm.
